The Contractor confirms location of work/access requirement/hours?

The Contractor has discussed vehicle access to work site (location/speed/seat-belts/PPE)?

The Contractor has been provided with farm contact numbers including emergence phone numbers and general enquiries phone numbers?

Have been advised of farm Drug and Alcohol policy?

The Contractor has the appropriate equipment for the work with all relevant safeguards in place?

If required Safe Work Method Statement (SWMS) for high-risk work (e.g. confined spaces, working at hights, trenching/evacuation) with risk assessment and detailed controls (if relevant) – (work will not commence until sighted).

The Contractor has a lock out procedure for plant and equipment?

All relevant PPE is to be provided by the contractor and used by workers?

Does the contractor have their own WHS policy and plan in place?  If yes, a copy needs to be provided to farm owner/manager.

Safe Work Method Statement (SWMS) document(s) with risk assessment and detailed controls sighted and discussed with farm owner/manager?

Discussed delivery/storage/removal of goods?

Discussed disconnection of utilities (if relevant)?

Incident reporting and investigation procedure for farms has been discussed?
